Position Summary

The Physical Therapist provides outpatient rehabilitation services in an in-home setting for older adults. Care is delivered in patients’ private residences or within independent living facilities, allowing therapy to occur in the most functional and meaningful environment—the patient’s home.

The Physical Therapist evaluates, develops, and implements individualized physical therapy programs to restore, reinforce, and enhance functional performance. This role includes patient and caregiver education, coordination with interdisciplinary teams, and referrals to other services as needed to address comprehensive patient care. The position offers a flexible schedule and varied work settings.


Key Responsibilities

  • Supervise Physical Therapy Assistants, students, temporary licensees, and support personnel in accordance with state licensure and professional standards.

  • Maintain adherence to patient schedules, documentation standards, billing timelines, and required meetings.

  • Participate in patient care conferences, staffing meetings, and interdisciplinary collaboration.

  • Identify patient needs and make referrals to appropriate disciplines or community resources.

  • Develop, adapt, and progress individualized plans of care based on patient response and goals.

  • Promote patient safety and maintain a hazard-free treatment environment.

  • Order, maintain, clean, and report repair needs for equipment and supplies.

  • Deliver patient-centered care with compassion, respect, and professionalism.


Clinical Duties

  • Perform comprehensive evaluations including patient history, systems review, testing, and interpretation of findings.

  • Establish diagnoses, identify impairments and precautions, determine prognosis, and create measurable treatment goals.

  • Implement and document physical therapy interventions, including but not limited to:

    • Therapeutic exercise (strength, endurance, balance, gait, posture, flexibility, neuromotor training).

    • Functional and mobility training.

    • Manual therapy techniques.

    • Airway clearance techniques.

    • Integumentary repair and protection.

    • Use of electrotherapeutic and physical agent modalities.

    • Fall risk assessments and prevention strategies.

    • Chronic disease management education.

    • Health and wellness programming.

    • Home and community case management.

  • Design, fit, and train patients in the use of adaptive equipment, orthotics, and prosthetics.

  • Conduct home and community safety and mobility assessments.

  • Reassess patients regularly and modify treatment plans as appropriate.


Education & Licensure Requirements

  • Valid Physical Therapy license in the state(s) of practice or proof of eligibility.

  • One of the following:

    • Master’s degree in Physical Therapy.

    • Master’s degree in a Physical Therapy–related field plus a Bachelor’s degree in Physical Therapy.

    • Bachelor’s degree in Physical Therapy with five (5) years of progressively responsible experience.

  • International degrees must be evaluated for U.S. equivalency.

  • Valid driver’s license and reliable transportation to patient residences.

  • Effective verbal and written communication skills.


Compliance Expectations

  • Adherence to all applicable laws, regulations, and organizational policies, including HIPAA and ethical standards.

  • Participation in required orientation, training, audits, and quality improvement initiatives.

  • Prompt reporting of compliance concerns through appropriate channels.


Physical & Work Requirements

  • Frequent standing and walking; occasional sitting.

  • Ability to lift, push, pull, and carry up to 50 pounds.

  • Regular bending, kneeling, stooping, climbing stairs, balancing, and working on uneven surfaces.

  • Exposure to noise, moving machinery, environmental hazards, and automotive equipment.

  • Occasional use of protective equipment (gloves, gowns, masks).

  • Work performed primarily indoors with limited outdoor exposure.
