Position Summary
The Occupational Therapist provides outpatient rehabilitation services in an in-home setting for the older adult population. Services are delivered in patients’ private residences or within independent living facilities, allowing therapy to occur in the most functional environment—the patient’s home.
The Occupational Therapist organizes and conducts occupational therapy programs to facilitate rehabilitation. This role includes directing patient participation in selected tasks to restore, reinforce, and enhance performance, as well as facilitating the learning of skills essential for adaptation, independence, and productivity. This position offers a flexible schedule and work environment across various settings.
Key Responsibilities
Supervise Occupational Therapy Assistants, temporary licensees, students, and support personnel in accordance with state licensure and professional standards
Maintain adherence to treatment schedules, documentation standards, billing timelines, and work-related meetings
Participate in patient care conferences, staffing meetings, and interdisciplinary collaboration
Make referrals to other disciplines and agencies to address comprehensive patient needs
Consult with rehabilitation team members, including community physicians, to determine appropriate therapeutic interventions
Adapt programs and plans of care based on individual patient needs
Promote a safe treatment environment and enforce safety standards
Order, clean, maintain, and report repair needs for equipment and supplies
Deliver patient-centered care with compassion and respect
Clinical Duties
Screen, examine, and evaluate patients through history, systems review, and appropriate assessments
Interpret evaluation findings to establish diagnoses, identify impairments, determine prognosis, and develop plans of care
Collaborate with patients and caregivers to establish treatment goals
Implement and document occupational therapy interventions, including but not limited to:
Cognitive skills training
Muscle strength, coordination, and endurance
Mobility and balance training
Perceptual and sensory awareness activities
Sitting and standing tolerance
Activities of daily living (ADLs)
Joint protection and work simplification techniques
Orientation and low vision interventions
Physical agent modalities
Fall risk management
Medication management support
Health and wellness education
Community integration and reintegration
Home and community case management
Design, fabricate, fit, and train patients in the use of adaptive devices, including orthotics and prosthetics
Select activities appropriate to the patient’s physical and cognitive abilities
Instruct patients in functional activity skills and evaluate progress and rehabilitative potential
Perform routine re-evaluations to modify or progress treatment plans
Perform other related duties as assigned
Education & Licensure Requirements
Valid Occupational Therapy license in the state(s) of practice or proof of eligibility
Valid driver’s license and reliable transportation to patient residences

Physical & Work Requirements
Physical Demands
Frequent standing and walking; occasional sitting
Ability to lift, push, pull, and carry up to 50 pounds
Regular bending, stooping, kneeling, crouching, crawling, twisting, balancing, and stair climbing
Work Environment
Primarily indoor, patient-home settings with limited outdoor exposure
Exposure to noise, environmental hazards, uneven surfaces, and automotive equipment
Occasional use of personal protective equipment (gowns, gloves, masks)
