Travel - Licensed Practical Nurse
POSITION SUMMARY
The Licensed Practical Nurse (LPN) functions as an experienced clinical team member, delivering skilled nursing care under the direction of a Registered Nurse (RN). This role requires sound clinical judgment, efficiency in patient care delivery, and the ability to supervise unlicensed staff while ensuring compliance with regulatory and facility standards.

Overtime pay : $65.45

CORE RESPONSIBILITIES
1. Clinical Assessment & Judgment
-
Performs focused patient assessments and monitors clinical status

Identifies, prioritizes, and reports significant changes in condition to the RN
-
Anticipates patient care needs and contributes clinically relevant input to care decisions
2. Care Planning & Outcome Management
-
Actively contributes to development, implementation, and revision of individualized care plans
-
Executes nursing interventions with minimal supervision
-
Evaluates patient response to treatment and escalates concerns appropriately
-
Supports strategies aimed at preventing avoidable re-hospitalizations
3. Skilled Nursing Care
-
Administers medications and treatments safely and efficiently
-
Performs skilled nursing procedures within scope of practice
-
Ensures timely, accurate clinical documentation in PointClickCare (PCC)
-
Communicates pertinent clinical information to RNs, physicians, and interdisciplinary team members
-
Assists with provider rounds and clinical evaluations as needed
4. Discharge & Transitional Care Support
-
Participates in interdisciplinary care conferences as directed
-
Reinforces discharge planning, patient education, and care transitions
-
Communicates care recommendations clearly to patients and families
5. Leadership & Oversight of Unlicensed Staff
-
Delegates and supervises patient care tasks in accordance with regulations and policy
-
Provides real-time guidance and follow-up
-
Holds unlicensed staff accountable for quality and completion of care
6. Interdisciplinary Collaboration
-
Serves as a consistent clinical resource to nursing assistants and peers
-
Participates in effective shift-to-shift handoff communication
-
Coordinates care across departments to support timely and patient-centered outcomes
7. Safety, Compliance & Professional Practice
-
Maintains medication security, including controlled substance counts
-
Adheres to infection control, HIPAA, and safety standards
-
Supports quality assurance and performance improvement initiatives
-
Participates in mandatory education and assists with onboarding of new staff
-
Reports compliance or safety concerns per facility policy
8. Operational Expectations

Demonstrates flexibility to meet patient care and staffing demands
-
Performs additional duties consistent with professional role and experience
REQUIRED QUALIFICATIONS
-
Graduate of an accredited practical or vocational nursing program
-
Active, unrestricted LPN license in the state of employment
-
Demonstrated experience in skilled nursing, rehabilitation, or geriatric care
-
Current BLS/CPR certification (per facility/state requirements)
-
Ability to communicate clearly and professionally in English
-
Compliance with TB screening and occupational health requirements
